As a subscriber in HMO Blue, Blue Choice, Blue Care Elect, HMO Blue Direct, HMO Basic Blue Direct, PPO Blue Direct, Blue Care 65 your Fitness Benefit could save your family up to $150 per year in health club membership dues or personal training fees. You can submit your health benefits claim once you've belonged to the Leading Edge for at least four months (in a calendar year).
