The Leading Edge: $19.95 agreement


	$19.95 Platinum Membership $29 activation fee. No Activation Fee ($29 Savings) -- On-line offer only, expires June 15th, 2010. This membership has a 12 month minimum term. Annual Maintenance Fee of $30 billed on or shortly after April 1st. If you join during the month of April, the annual maintenance will be will collected at the time of joining. BILLING FOR MONTHLY DUES: By signing this agreement, I have authorized The Leading Edge/Twin Oaks to bill my bank account or credit card for monthly dues for a minimum of a 12-month commitment period. If Twin Oaks is unable to collect my monthly dues for any reason, Twin Oaks will impose an administration fee of $10. A once a year maintenance fee of $30 will be assessed to each E.F.T. membership every April 1st. If you join during the month of April, the annual maintenance will be will collected at the time of joining. 2. WITHDRAWAL FROM DUES/TERMS: After a minimum 12-month commitment period, billing will continue until written notice is given to cancel. Cancellations can be done upon completion of contract by sending a certified letter to THE LEADING EDGE Cancellation Dept., 278 Mohawk Trail, Greenfield MA 01301. A 1 (one) month notice is required prior to the billing date to cancel. The Leading Edge reserves the right to terminate a membership due to a violation of club policies. The Leading Edge Contract: RIGHTS OF CANCELLATION - THREE (3) DAY CANCELLATION POLICY "CONSUMER’S RIGHT TO CANCELLATION. YOU MAY CANCEL THIS CONTRACT WITHOUT PENALTY OR FURTHER OBLIGATION BY CAUSING A WRITTEN NOTICE OF YOUR CANCELLATION TO BE DELIVERED IN PERSON OR POSTMARKED BY CERTIFIED OR REGISTERED UNITED STATES MAIL WITHIN (3) BUSINESS DAYS OF THE DATE OF THIS CONTRACT, OR THE DATE OF YOUR RECEIPT TO THE ADDRESS SPECIFIED IN THIS CONTRACT.” You may also cancel this contract for any reason provided that, at the time of your cancellation notice, you enclose a valid check or money order, or you authorize in writing a debit to a valid credit card already on file with The Leading Edge (from which monthly dues are taken) a sum equal to THREE MONTHS membership dues (or the balance of contract dues, whichever is less); and provided also that your membership account with The Leading Edge is current in all obligations, and provided that notice of such cancellation follows the requirements set out above. Member agrees to follow club rules as stated. Violation of these rules may be the cause for suspension or cancellation of membership. Any holder of this consumer credit contract is subject to all claims and defenses which the buyer/member could assert against the club as a result of this contract. Recovery by the buyer/member shall not exceed the total amount paid by the buyer/member to the club pursuant to this contract. To the best of my knowledge, I do not have any physical limitations that would prevent me from participating in the program. I also understand that I participate in said program at my own risk.
